Fraser Health's iConnect Health Network's Community Wellness initiative provided the
Stroke Recovery Association of BC (SRABC) with a grant to establish key community
linkages and provide resources to community living stroke survivors. The intent of this
project is to enhance SRABC's services by creating an integrated community resource
network for stroke survivors and their caregivers living in the Surrey and White Rock
area. The iConnect Community Linkage Coordinator, Cindy Dalglish, is working to
continue the Association's vision for every stroke survivor to have respect, inclusion, and
support in their home community.
 

**This project is well underway and will be completed by the end of April. Our Branch Coordinators will be trained to carry this project forward in maintaining the database and in referring our membership to the resources.


The goals of this project are:
  •  To identify the community resources stroke survivors need by meeting with
    stroke survivors and caregivers at the branch level. Some examples include,
    mental health services, social and recreation programs, peer support, etc.
  • To develop a resource directory of the community resources found for stroke
    survivors, caregivers and Branch Coordinators
  • To strengthen the link between branches in Surrey and White Rock and providers
    of stroke care in Fraser Health by linking to the iConnect Health Centres and
    community partners

 

The overall goal of this project is to assist our Branch Coordinators in empowering the
stroke survivor through knowledge and access to available community programming.
Upon success of this project, SRABC and Branch Coordinators will have the necessary
tools to expand the Community Linkage Program to other branches throughout BC.
